Our Services
Daily Personal Care
Personal care supports relate to assistance with daily personal activities including assistance with, or supervision of, personal tasks of daily life. This may be required across a variety of settings.
Transportation
Hope & Home Care Community has its own transport vehicle for many passengers or for individual use. Our licensed support workers are always ready to provide safe transportation.
Transitional Care
We provide time-limited, goal-oriented therapy-focused packages of services to people after a hospital stay to help recovery and re-establish independence. Services may include – Nursing support.
Community Participation
We assist and encourage individuals who wish to participate in the wider community. This includes- Programs at local councils, Library services, Youth group participation, Religious and Social Justice events.
Domestic/Household Tasks
We can provide assistance with essential tasks that the participant is not able to undertake such as meal preparation, and delivery, house or yard maintenance, cleaning and linen. This will be achieved through assistance.
